ADMISSION and ENROLLMENT

Children are accepted to the center both by private referral and by Family Central, Inc., for families requiring financial assistance with childcare expenses.  Vacant slots are filled on a first-come-first-served basis without regard to race, creed, color or national and ethnic origin.  Parents must have a genuine desire to have their child(ren) attend and become a part of the Turner’s Kindergarten family.  Parents must agree to cooperate fully with the staff and administration, and abide by all school policies and programs.  The State of Florida determines the center’s maximum student capacity.

ENROLLMENT PROCEDURE

Enrollment in Turner’s Kindergarten will be finalized upon completion of the following steps:

1. An enrollment form signed and dated by at least one parent or guardian
2. a current medical examination (DCF 3040 f0rm), tuberculosis test (PPD) and results, and an immunization record (DCF 6980) from the child’s private doctor or any public health facility,
3. A copy of the child’s birth certificate and a copy of their social security card,
4. Student information form that includes, but not limited to:

   name of parent or legal guardian’s address, and working telephone numbers for home, cell or beeper.

   name of doctor and hospital preference.

   authorized person(s) to pick-up child(ren).

   emergency contact persons with current telephone number.

   work place and telephone number.

5. Payment of all applicable fees
6. Complete a food service application
7. Food stamp authorization number, if applicable.

Address or telephone number changes:  Any time your address or telephone number (home/work) is changed, the school must be notified immediately.
